In a recent Fortune article, our CEO, Carrie, shared her journey in a piece titled “Cancer Made Me a Patient. An ordinary Tuesday, an extraordinary shift

For Carrie, the journey that would eventually redefine her life and career began on an otherwise ordinary Tuesday. At 34, she received the devastating call that no one expects: a diagnosis of breast cancer. As a single mother and a dedicated Emergency Medicine Physician Assistant, Carrie suddenly found herself confronting not only the shock of her illness but also the daunting path ahead—balancing her career, her role as a mother, and the fight for her life.

This experience wasn’t just a personal challenge; it revealed to Carrie the stark realities of healthcare’s critical gaps. She quickly saw how disconnected the system could be, leaving patients to manage alone in the most vulnerable moments. It was during these times, facing her own fears and needs, that she recognized just how much patients like her needed comprehensive, compassionate care that didn’t end at the hospital doors.

One of the most poignant moments came when her young daughter traced the scars on her chest and asked if they hurt. After a post-surgery infection landed her back in the hospital, she saw firsthand the gaps in post-discharge care and the steep financial and emotional toll they take. She’s not alone: studies show that nearly 20% of Medicare patients are readmitted to the hospital within 30 days, with readmissions costing the U.S. healthcare system approximately $62 billion every year.

The problem goes beyond just cost. Lack of adequate post-discharge care can significantly impact patient outcomes and quality of life. A study by the Journal of the American Medical Association found that patients who received timely follow-up care after discharge had a 40% lower chance of being readmitted. Yet, despite the importance of this transition, many patients are left without sufficient support during this critical recovery period.

After 17 years in healthcare, Carrie was no stranger to the challenges of patient care. But it was her own experiences as a patient that exposed what she saw as a critical flaw in the healthcare system: the risky gap between hospital discharge and home. Determined to address this gap, Carrie joined forces with her co-founders, Gidon Coussin , Dhaval Patel and Sarig Reichert, to launch Dimer Health—a solution built on the concept of continuous, proactive care for patients after discharge. Together, they designed an approach that goes beyond traditional models, integrating 24/7 support and follow-up to help patients navigate recovery smoothly and confidently.
